Defibrillators: A ‘must have’ ambulance device

Whenever the heart beat goes insane alarm bells start ringing. They, in most cases, lead to fatalities. In such a situation a defibrillator is used to calm things down and bring the heart back on track. A healthy human heart beats about 60 times a minute, that’s 1 beat a second. This means that the heart opens and closes once every second, pumping blood in and out. But what if the rate increases? Now the heart has to open and close even faster. A time comes when the rate becomes so fast that the heart doesn’t get enough time to pump blood out. Furthermore it must be understood that blood flow path is guided by electrical signals. But sometimes a sudden impulse leads to irregularities that block the path of blood circulation. This leads to drop in cardiac output called as Ventricular fibrillation or VF. This is pretty much the scenario which necessitates use of defibrillators.

By sending a strong electrical shock wave, defibrillator forces each cardiac cell to ‘return’ to their original states thus restoring the heart rate. There is, of course, a gap of few seconds between application of shock and emergence of normal heart beat. Traditionally used only in ambulances and hospitals by doctors, they have become a common medical equipment at almost every venue, whether it be playground, hospital, schools etc. But perhaps it’s most important application is seen in ambulances especially the ones suited for life-threatening conditions like MICU or ICU ambulance services. What is a basic life support or BLS?

An emergency can strike anywhere and anytime, sometimes they can be life-threatening and sometimes they can be less serious. And sometimes it’s just about getting an inter-hospital transfer. But regardless of the degree of severity, a  basic life support ambulance or personnel is always needed to provide you with timely and quick medical support. But what exactly is a BLS? BLS is not same as ALS!

To begin with, a Basic life support is one which is intended to provide first medical response to anyone in need. They are different from Advanced life support because unlike them, who are allowed and equipped to administer drugs, a BLS personnel is neither equipped nor allowed to administer drugs and are not trained with invasive skills. Also, while an ALS can be provided only by a paramedic or a doctor, a BLS is performed mainly by the first responders or FR. Even the bystanders at possible emergency places like school teachers at school, a passerby at the site of accident etc. can provide basic life support if they know a bit about first aid.

The basic idea of BLS – follow CAB!

While the medical procedures, sequences and algorithms are prescribed and prepared by medical professionals, the basic tenet is rather simple – CAB i.e. maintaining circulation of blood by bleeding control and CPR; keeping airway open either manually or by using nasal or oral adjuncts; and facilitating breathing by means of artificial respiration. Different countries have their own procedures. For instance, in USA, a bystander can perform BLS if he (or she) knows steps of resuscitation known as DRS-CABCDE (abbreviation of Danger, Response and Shout for help – Catastrophic hemorrhage, Airway, Breathing, Circulation, identify disabilities and environment.)

A BLS ambulance needs some basic equipment…and drugs.

As far as a Basic life support ambulance is concerned, the design and requirements vary from place to place. Nonetheless, there are some equipment and drugs that are common to all of them. These include Automatic external defibrillator (AED), stretchers, suction devices, big mask ventilation units, splints, bedpan etc.
